NHS England - List Maintenance Project 2015

Registration letters from NHS England – please respond if you get one

The NHS is carrying out work in Bath, Gloucestershire, Swindon and Wiltshire to make sure the patient details on GP lists are accurate.  Letters are being sent to some patients, but not all. PATIENT ONLINE ACCESS TO MEDICAL RECORDS

STATEMENT OF INTENT

Spa Medical Centre now offer patients all of the requirements under the GP contract relating to online services and are working closely with our system supplier (SystmOne) to allow patients to access more detailed information from their medical records.  It is our intention to have this available by 31st March 2016

If you would like to sign up to all the online services we provide, please fill in the "Online Access Application Form" and hand it in to Reception.  Please remember to bring identification with you such as passport, driving licence, birth certificate, bank statement, utility bill.

Duty Team - Same Day Appointments

blood-pressureOur Duty Doctor and Nurse are happy to see patients with urgent problems that require immediate attention. You will be asked some brief questions about the nature of your problem and, although you are not obliged to tell us, it helps us decide whether you need to see a GP, Nurse or other healthcare professional. The Receptionist can help you decide which healthcare professional is most appropriate for your needs, or whether pharmacy or other agencies can offer you the help and support required.

Any information given to staff is treated in the strictest confidence as they are bound by the same rules of confidentiality as the Doctors and Nurses.

A morning and afternoon surgery is available for patients who have problems that have arisen suddenly and consider the need too pressing to wait for a routine appointment. Emergency appointments are allocated in chronological order, some patients may be seen sooner due to clinical need.  Where the Duty Doctor may be called out on an emergency visit, patients may have to wait more than usual to see the doctor. Our reception staff will advise you of this should the situation arise.Please bear with us in these situations, we will endeavour to see you as soon as is practically possible.
