Registering as a new patient

If you wish to register at Spa Medical Centre, please call in to reception to collect a registration pack.

In the pack will be a Family Doctor Registration Form (GMS1) along with other forms and leaflets for you to read and fill in. Please make sure you read everything carefully and fill in the forms as clearly and as fully as possible.

You will need one pack per person registering.

Identification

All new patients are required to provide 2 forms of identification, ideally one from each of the following groups:

  • Passport, Photo Driving Licence or Birth Certificate.
  • Utility Bill, Bank Statement, Letter from Benefits Agency/HMRC.

Please return the completed forms to the Practice, ideally after 4.30pm. Your forms will then be checked with you and at this point we will also check proof of your identity.

Please note that if you are 18 or over, you must attend in person so we can check your ID.

Once the forms have been checked, it can take up to 48 hours to register you on our system so if you require urgent medical care, please mention this to the Reception team who will do their best to help.

Repeat Medication

If you are on repeat medication you will be required to book a routine appointment with a GP. The waiting list for these appointments could be 3-4 weeks so please ensure you have sufficient medication from your previous Practice before completing the registration process with us. Please also provide us with your current repeat prescription slip if possible, so that this can be added to your records.

Online Services

If you would like to sign up for online services, click here for our registration form (or collect one from the surgery).

Once your form has been completed, please bring this to the surgery together with ID. We need 1 piece of photographic ID, e.g your Driving Licence or Passport and 1 piece of proof of address, such as a utility bill.
