Registration letters from NHS England – please respond if you get one

The NHS is carrying out work in Bath, Gloucestershire, Swindon and Wiltshire to make sure the patient details on GP lists are accurate.  Letters are being sent to some patients, but not all. If you do receive a letter, please either confirm your details or submit any changes. For more information about this project please visit



Spa Medical Centre now offer patients all of the requirements under the GP contract relating to online services and are working closely with our system supplier (SystmOne) to allow patients to access more detailed information from their medical records.  It is our intention to have this available by 31st March 2016

If you would like to sign up to all the online services we provide, please fill in the "Online Access Application Form" and hand it in to Reception.  Please remember to bring identification with you such as passport, driving licence, birth certificate, bank statement, utility bill.

 Registering as a new patient

If you wish to register at Spa Medical Centre, we ask that, for each person registering, you come and collect a registration pack from the surgery.

In the pack will be a Family Doctor Registration Form (GMS1) along with other forms and leaflets for you to read and fill in.  Please make sure you read everything carefully and fill in the forms as clearly and as fully as possible.


All new patients are required to provide 2 forms of identification, ideally one from each of the following groups:

 1) Passport, Photo Driving Licence or Birth Certificate.

2) Utility Bill, Bank Statement, Letter from Benefits Agency/HMRC.

Please return the completed forms to the Practice, ideally between 1pm and 2pm or after 4.30pm. Your forms will then be checked with you, and at this point we will also check proof of your identity.

Once the forms have been checked, it can take up to 48 hours to register you on our system. It can take several weeks to receive your paper records from your previous GP so if you require urgent medical care, please mention this to the Reception team who will do their best to help.

 Repeat Medication

If you are on repeat medication you will be required to book a routine appointment with a GP. The waiting list for these appointments could be 3-4 weeks so please ensure you have sufficient medication from your previous Practice before completing the registration process with us. Please also provide us with your current repeat prescription slip if possible, so that this can be added to your records.

Online Services

If you would like to sign up for online services, please request an application form from reception.
